Case Management

We support individuals returning home from incarceration by providing coordinated, person-centered care that addresses the full scope of reentry needs. Through individualized planning, participants are connected to essential resources such as housing, employment, education, mental health services, and community support networks. Our approach centers dignity, accountability, and long-term stability, ensuring each person has consistent guidance as they rebuild their lives, strengthen their support systems, and successfully reintegrate into their communities.

Our community health coaches are certified as Community Health Workers, Peer Support Specialists, Violence Prevention Professionals, and Career Coaches. This training is designed to prepare frontline staff to work effectively with individuals and communities facing complex health, behavioral health, and social needs.
